We strive to make your visit to our office as smooth and efficient as possible. Here’s what to expect before, during, and after your appointment.
Before Your Appointment
Mobile Check-In
We’ll send you a link to our mobile check-in app, APPatient, where you will update or confirm your medical history and medications. Completing this before your appointment will help minimize your wait time and maximize your time with your provider.
In-Office Kiosk Check-In
If you don’t have a smartphone or prefer not to use the app, you must check-in and update your medical history and medications at the kiosks in the lobby. We recommend arriving a few minutes early if you plan to use a kiosk. Our staff will be available to assist you with the check-in process as needed.
Notify Staff
Upon arriving for your appointment, please notify our front desk so we can let your provider know you are here for your appointment. Please note that a photo ID is required for us to see you.
During Your Appointment
After checking in through the mobile app or at a kiosk, one of our medical assistants will welcome you and guide you to the exam room. They’ll confirm your medical information, take a photo for your patient chart, and make sure everything is ready for your appointment with your provider.
Highly Trained Dermatological Providers
Our medical staff consists of highly trained Physician Assistants (PAs) and Nurse Practitioners (NPs) specializing in dermatology. They have all undergone extensive training directly with Dr. Michael Nazareth, who is always on-site as the supervising physician. Meet our team.
Dr. Michael Nazareth, Supervising Physician
As the supervising physician, Dr. Nazareth ensures that our physician assistants and nurse practitioners provide the highest level of care. By overseeing the entire practice, he facilitates medical decisions, offers guidance and support, and monitors each patient’s progress.
Meeting with Your Provider
Your appointment with your provider will last approximately 15 minutes. During this time, we’ll conduct a skin check, review your treatment progress, and address any concerns you may have. Please note that some skin conditions may require multiple or separate appointments to address fully. Your provider will discuss a personalized treatment plan with you, and a medical assistant will assist in scheduling any necessary follow-up appointments.

After Your Appointment
If your skin condition requires a follow-up, we will discuss this and schedule it during your appointment. Please complete any lab work that was asked for and continue to follow your treatment plan as your provider instructed.
Patient Portal and Biopsy Results
Your patient portal will be updated with your biopsy results. If your biopsy results indicate that further treatment is needed for your skin condition, we will contact you to schedule a follow-up appointment or provide you with referral information. Otherwise, you can review your results through the online patient portal or the APPatient app.
